I/We, (name of parent) hereby give my consent for my son / daughter to participate in the above mentioned competition.I/We agree that though the organizer will make every reasonable effort within their scope of duties to ensure safety and well-being of my child, I/we realize that even under close supervision, children may have occasional accidents. Therefore, we hereby release for indemnity and hold UCMAS USA Inc, its franchisee and employees harmless from any and all claims, damages or other liabilities for injuries to my child which are notresult of direct negligence of the staff.
I/We further agree that in spite of being extremely careful and vigilant about the quality of PA and audio/video system for the listening/flash competition there might be certain uncontrollable disturbances in the audio or video output and we hereby release for indemnity and hold UCMAS free from any responsibilities caused due to such uncontrollable circumstances.We grant permission to the organizers of the event to use photographs and visual recordings of my child taken in this event, provided that no identification (name or address) will be used unless explicitly authorized.
